SprintFeed: HSE forms

Build HSE forms in the Toolbox, see how workers submit them from site, and review submissions on Form responses.

HSE forms are checklists and data capture that workers fill in on site through the app. Each submission is stored for reviewers and emailed to the addresses you choose.

Build a form

Admins and anyone with Can manage Toolbox can build forms.

  1. Open Toolbox, go to the right section, and click Add form.

  2. Enter a Title (required) and an optional description.

  3. Click Add field for each question, pick its type, and tick Required where needed.

  4. Open Form settings and add Notify by email addresses. Without any, submissions are still stored but nobody is emailed.

  5. Set Visible to locations, or leave it empty for everyone.

  6. Click Save form. It's live straight away unless Hide from Toolbox is on.

Photo

Take or choose a photo. It must finish uploading before Submit.

Coordinator

Pick a person from the SprintSuite staff list, filtered by role. Their email is added to the notification list for that submission.

Signature fields and sections within a form aren't supported. Group related forms using Toolbox sections instead.

Other form settings

  • Confirmation message: the text shown after Submit. The default is Thanks, your response has been sent.

  • Show/hide rules: show a field only when an earlier field has a set value. The trigger field must sit above the one it controls.

  • Hide from Toolbox: only admins and Toolbox managers see the form while you test.

How workers submit

  1. In the app, open Toolbox, tap into the section, then tap the Form tile.

  2. Answer each question. Required fields show a red asterisk.

  3. Tap Submit. Missing answers are flagged straight away.

Workers need a signal to submit. Forms don't save drafts or queue offline, so anyone who leaves part-way has to start again.

Review submissions

Open Form responses in the sidebar. Admins see it by default. Others need Can review form responses, which is separate from Can manage Toolbox.

Click a row or Show answers to see the full response, including photo links. Rows are newest first. There's no search, export or print on this page.

  • Resend sends again, only to the addresses that failed.

  • Mark as read clears a failed row from the count when no action is needed.

The red badge on Form responses counts failed emails that haven't been resent or marked as read. It isn't a count of new submissions.

Editing a live form

Old submissions keep their original answers. Labels on Form responses come from the current form, so renaming a question changes how older rows read. Moving a form to Trash stops new submissions, but its past responses stay in Form responses.
